On 9 November 2022, a lab assistant sustained serious injury when she was struck by a forklift in the bottling line area.
After a SafeWork NSW investigation the defendants, The Real Juice Company Pty Ltd, and the Director, Mr Anthony Taliano, were respectively charged with a breach of sections 32/19(1) and sections 32/27(1) of the Work Health and Safety Act 2011.
On 26 June 2026 the defendants were convicted by the Industrial Court of NSW, and the corporate defendant was fined $240,000 and made subject to a publicity order pursuant to s236(a)(i) and the director defendant was fined $9,600.
Mulligan Geotechnical Pty Ltd

On 15 March 2023, a worker sustained serious fractures when he fell approximately 11 metres from the bucket of a Boom Lift, whilst undertaking embankment remediation works.
After a SafeWork NSW investigation the defendant, Mulligan Geotechnical Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 24 June 2026, the defendant was convicted by the Industrial Court of NSW and fined $225,000.
Solution Hoist Hire Pty Ltd

On 21 April 2023, a worker was injured when he was converting an electric hoist’s power supply from a trailing cable system to a busbar system. The worker’s arm came into contact with the energised busbar, suffering an electric shock and serious burns.
After a SafeWork NSW investigation the defendant, Solution Hoist Hire Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 16 June 2026, the defendant was convicted by the Industrial Court of NSW and fined $360,000.
Capital Lines & Signs Pty Ltd

On 3 February 2023, a worker was seriously injured when she was struck by hot bitumen that was discharged from a vehicle-mounted melter.
After a SafeWork NSW investigation the defendant, Capital Lines & Signs Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 16 June 2026, the defendant was convicted by the Industrial Court of NSW and fined $120,000.

The defendant was charged with a breach of Clause 79(2) of the Work Health and Safety Regulation 2017.On 28 May 2026, the defendant was convicted by the Downing Centre Local Court and fined $1,000.

On 2 September 2022 a woman and three small children sustained injuries when a sliding gate overran its supports and fell onto them.
After a SafeWork NSW investigation the defendant, Hanna Nsaire, was charged with a breach of sections 32/27(1) of the Work Health and Safety Act 2011.
On 27 May 2026, the defendant was convicted by the Industrial Court of NSW and fined $50,000.
Ausino Group Pty Ltd and Hong (Robin) Sang

On 29 September 2022, a mobile scaffold was blown over a rooftop handrailing and fell six stories onto the glass roof of the neighbouring property.
After a SafeWork NSW investigation the defendants, Ausino Group Pty Ltd, and its Director, Hong (Robin) Sang, were respectively charged with a breach of sections 33/19(2) and sections 33/27(1) of the Work Health and Safety Act 2011.
On 15 May 2026 the defendants were convicted by the Industrial Court of NSW, and the corporate defendant was fined $20,000 and the director defendant was fined $8,000.

On 2 February 2022, a female NDIS participant sustained serious burns when she was placed in a bath with water at an excessive temperature and died from her injuries.
After a SafeWork NSW investigation the defendant, LiveBetter Services Limited, was charged with a breach of sections 32/19(2) of the Work Health and Safety Act 2011.
On 1 May 2026, the defendant was convicted by the District Court and fined $675,000.

On 8 July 2022, a worker was crushed by the roller of a Van Gelder Waste Grinder machine when he entered it to clear a blockage.
After a SafeWork NSW investigation the defendant, Northwest Recycling Centre Pty Ltd , was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 1 May 2026, the defendant was convicted by the District Court and fined $472,500.

On 1 July 2021, a worker was seriously injured while working on the first floor of a residential building under construction when he fell approximately 3 metres over an unprotected edge to the ground below.
After a SafeWork NSW investigation the defendants, Acon Projects Pty Ltd, and the Director, Adrian Conditsis, were respectively charged with a breach of sections 31/19(1) and sections 32/27(1) of the Work Health and Safety Act 2011.
On 30 April 2026 the defendants were convicted by the District Court and the corporate defendant was fined $480,000 and the director defendant was fined $20,000.

On 25 July 2022 a worker sustained injuries when he fell approximately 4 metres from scaffolding.
After a SafeWork NSW investigation the defendant, Customconstruction Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 1 April 2026, the defendant was convicted by the District Court and fined $100,000.

On 24 March 2026, the defendant was convicted by the Industrial Court of NSW and fined $70,000.

On 10 October 2022, a worker was walking at the front of a forklift carrying a door. The forklift caught the workers foot running over her legs resulting in serious injury.

On 22 May 2021, three workers were cleaning a roof above a retail store when a worker tripped on a sky light and fell approximately 10 metres to the ground causing serious injuries.
After a SafeWork NSW investigation the defendant, PHBC LH Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 24 February 2026, the defendant was convicted by the District Court and fined $270,000.

On 22 May 2021, three workers were cleaning a roof above a retail store when a worker tripped on a sky light and fell approximately 10 metres to the ground causing serious injuries.
After a SafeWork NSW investigation the defendant, Pendle Ham & Bacon Pty Ltd, was charged with a breach of sections 32/19(1) and 32/20(2) of the Work Health and Safety Act 2011.
On 24 February 2026, the defendant was convicted by the District Court and fined $270,000 and $50,000 respectively.

On 12 February 2022, a labour hire worker was operating a motorised pallet rider when he collided with another pallet rider, and his foot sustained a crush injury.

On 9 February 2026, the defendant was convicted by the District Court and fined $125,000.
