After a SafeWork NSW investigation the defendant, General Forest Tree Surgeon Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 24 March 2026, the defendant was convicted by the Industrial Court of NSW and fined $70,000.
Steel-Line Garage Doors Australia Pty Ltd

On 10 October 2022, a worker was walking at the front of a forklift carrying a door. The forklift caught the workers foot running over her legs resulting in serious injury.
After a SafeWork NSW investigation the defendant, Steel-Line Garage Doors Australia Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 17 March 2026, the defendant was convicted by the Industrial Court of NSW and fined $450,000.

On 22 March 2022, a maintenance worker fell approximately 5.7m through a skylight on the roof of a grandstand into the foyer below.
After a SafeWork NSW investigation the defendant, The Illawarra Turf Club Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 26 February 2026, the defendant was convicted by the District Court and fined $200,000.
PHBC LH Pty Ltd

On 22 May 2021, three workers were cleaning a roof above a retail store when a worker tripped on a sky light and fell approximately 10 metres to the ground causing serious injuries.
After a SafeWork NSW investigation the defendant, PHBC LH Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 24 February 2026, the defendant was convicted by the District Court and fined $270,000.

On 22 May 2021, three workers were cleaning a roof above a retail store when a worker tripped on a sky light and fell approximately 10 metres to the ground causing serious injuries.
After a SafeWork NSW investigation the defendant, Pendle Ham & Bacon Pty Ltd, was charged with a breach of sections 32/19(1) and 32/20(2) of the Work Health and Safety Act 2011.
On 24 February 2026, the defendant was convicted by the District Court and fined $270,000 and $50,000 respectively.

On 5 October 2022, a worker stepped onto a stationery outfeed conveyor. Another worker turned it on causing him to be caught between the conveyor rollers and moving trolley.
After a SafeWork NSW investigation the defendant, Painting and Blasting Industries Pty Ltd, was charged with a breach of sections 32/19(1) of the Work Health and Safety Act 2011.
On 13 February 2026, the defendant was convicted by the Industrial Court of NSW and fined $120,000.

On 12 February 2022, a labour hire worker was operating a motorised pallet rider when he collided with another pallet rider, and his foot sustained a crush injury.
After a SafeWork NSW investigation the defendant, Momentum Consulting Group Pty Ltd, was charged with a breach of sections 33/19(1) of the Work Health and Safety Act 2011.
On 9 February 2026, the defendant was convicted by the District Court and fined $125,000.

On 5 December 2022, a piling rig that was lifting a 16m steel reinforcement cage, has manoeuvred the cage into 11KVA overhead power lines.
