On 22 February 2016, a 45 year old painter was injured when he fell from a height.

After a SafeWork NSW investigation, the defendant, Miro Maric, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 15 April 2019, the defendant was convicted by the District Court and fined $37,500.

MJM Painting Services Pty Ltd

After a SafeWork NSW investigation, the defendant, MJM Painting Services Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 15 April 2019, the defendant was convicted by the District Court and fined $187,500.

Roland Albert Samuels

On 14 October 2016, a worker in Dubbo suffered a deep laceration to his hand when his clothing became caught on, and he was drawn into, an unguarded rotating shaft of a chaff cutting machine.

After a SafeWork NSW investigation, the defendant, Roland Samuels, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 10 April 2019, the defendant was convicted by the District Court, fined $22,500, ordered to undertake training under section 241 of the Act, ordered to conduct a work health and safety project pursuant to section 238 of the Act, and to enter into a work health and safety undertaking under section 239 of the Act.

Macquarie Milling Co Pty Ltd

The defendant, DSD Builders Pty Ltd, was charged with a breach of regulation 79(2) of the Work Health and Safety Regulation 2017.

On 29 June 2016, a 27 year old mechanic in Eastern Creek, suffered serious crush injuries when he was working underneath a tipper truck and the bucket lowered, trapping him.

After a SafeWork NSW investigation, the defendant, Haines Bros Earthmoving Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 22 March 2019, the defendant was convicted by the District Court and fined $90,000.

Grey Colt Pty Ltd

On 1 July 2016 (notified 23 August 2016), a 17 year old kitchen hand suffered third degree chemical burns to his feet after cleaning up a liquid substance that was on the floor.

After a SafeWork NSW investigation, the defendant, Grey Colt Pty Ltd, was charged with a breach of section 32/19(1) and section 38 of the Work Health and Safety Act 2011.

On 22 March 2019, the defendant was convicted by the District Court and fined $22,500 under section 32/19(1) of the Act and $1,500 under section 38 of the Act.

QSR (t/a KFC Coffs Harbour) Pty Ltd

On 17 November 2016, a cook at KFC suffered serious burn injuries to his left leg when he slipped in the kitchen while cleaning.

After a SafeWork NSW investigation, the defendant, QSR Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 22 March 2019, the defendant was convicted by the District Court and fined $60,000.

On 22 September 2014, a 20 year old contract musterer suffered fatal head injuries when he fell from the motorbike he was using to muster sheep.

After a SafeWork NSW investigation, the defendant, KD & JT Westbrook Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

O 19 February 2019, the defendant was convicted by the District Court, fined $180,000 and ordered to publish the orders under s236 of the Adverse Publicity Order.

Building Maintenance Unit Service Pty Ltd

On 20 October 2015, two window cleaners were seriously injured when the building maintenance unit (BMU) they were using to clean the external windows of the building failed and they fell from the 12th floor.

After a SafeWork NSW investigation the defendant, Building Maintenance Unit Service Pty Ltd, was charged with a breach of section 32/19(2) of the Work Health and Safety Act 2011.

On 18 February 2019, the defendant was convicted by the District Court and fined $300,000.
