On 5 December 2017, a truck driver was fatally injured when a large load of stacked timber being unloaded from his truck by a forklift shifted and fell to the ground, striking him, at Campbelltown.
After a SafeWork NSW investigation, the corporate defendant, Campbelltown Hardware Pty Ltd, and the director, Muhammad Jawad Khalid, were respectively charged with a breach of section 32/19(2) and section 32/27(1) of the Work Health and Safety Act 2011 (the Act).
On 31 May 2021, the defendants were convicted by the District Court. The corporate defendant was fined $75,000, and the director was ordered to enter into aWork Health and Safetyundertaking pursuant to section 239 of the Act for a period of 2 years.
Concrite Pty Ltd

On 20 September 2017, a 61 year old male truck driver was fatally injured when he walked into the path of, and was struck by, a concrete mixing truck, at work site located in Alexandria.
After a SafeWork NSW investigation, the defendant, Concrite Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 21 May 2021, the defendant was convicted by the District Court and fined $450,000.
Sandhu Construction Group Pty Ltd

On 16 August 2017, a subcontractor sustained serious injuries when a masonry brick wall being constructed collapsed on to him while he was clearing the alfresco area of the Kellyville construction site.
After a SafeWork NSW investigation, the defendant, Sandhu Construction Group Pty Ltd was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
The defendant was convicted by the District Court and on 21 May 2021 was fined $665,000.
Byford Equipment Pty Ltd

On 13 February 2018, an 18 year old apprentice was rendered unconscious after being overcome by the fumes of hazardous chemicals while working in a confined space of a stainless steel tanker, at Moama.
After a SafeWork NSW investigation, the defendant, Byford Equipment Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 12 May 2021, the defendant was convicted by the District Court and fined $30,000.
Steven Bruce Mudford

On 29 November 2017, a visitor who was assisting in a shearing shed suffered serious injuries to her scalp when her hair got caught in a rotating shaft of the overhead drive of sheep shearing gear, at Gulargambone.
After a SafeWork NSW investigation, the defendant, Steven Mudford, was charged with a breach of section 32/19(2) of the Work Health and Safety Act 2011.
On 7 May 2021, the defendant was convicted by the Local Court and fined $16,500.
