On 1 February 2018, a contractor was injured when he was struck by high pressure water while removing a spacer between two pipes, at the M4 East Motorway, Five Dock.
After a SafeWork NSW investigation, the defendant, M G W Engineering Pty Limited, was charged with a breach of section 32/19(2) of the Work Health and Safety Act 2011.
On 23 July 2021, the defendant was convicted by the District Court and fined $112,500.

On 28 May 2018, a 25 year old service mechanic suffered significant burns to his body when the scissor lift he was operating extended into overhead wires, at St Mary’s.
After a SafeWork NSW investigation, the defendant, Onsite Rental Group Operations Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act2011.
On 23 July 2021, the defendant was convicted by the District Court and fined $150,000.

On 11 September 2018, a worker sustained serious injuries when 700kg of glass panels being transported in A-Frame trolley by a forklift fell onto him, at Arndell Park.
After a SafeWork NSW investigation, the defendant, Rex Australia Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 23 July 2021, the defendant was convicted by the District Court and fined $225,000.

On 23 October 2017, a 27 year old worker suffered fatal injuries when the truck he was working underneath was displaced and travelled down a slope, at Termeil.
After a SafeWork NSW investigation, the defendant, South Coast Under Road Borers Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 14 July 2021 the defendant was convicted by the District Court and fined $375,000.

On 18 February 2019, a 20 year old 1st year apprentice carpenter was seriously injured when his hand was crushed after it was caught between the hoist cable and sheave in a hook block of a tower crane being operated at a site in Gosford.
After a SafeWork NSW investigation, the defendant, Aptec Constructions Pty Ltd was charged with a breach of section 38(1) and section 39(1) of the Work Health and Safety Act 2011.
On 12 July 2021, the defendant was convicted by the Local Court and fined $15,000 for each of the offences under sections 38(1) and 39(1) of the Act.
