On 17 November 2018, an air-conditioning mechanic sustained fatal injuries when he fell approximately 3 metres through a second floor void at a construction site in Green Valley.
After a SafeWork NSW investigation, the defendants, Milia Hanna and Anthony Hanna, were respectively charged with a breach of section 32/19(1) and section 32/28(b) of the Work Health and Safety Act 2011.
On 21 December 2021, the defendants, were convicted by the District Court, fined $150,000 under section 32/19(1) and $75,000 under section 32/28(b) for their respective offences, and ordered to undertake a work health and safety project pursuant to section 238 of the Act.
Camden Council

On 3 July 2018, a volunteer from Men’s Shed volunteer organisation was laying pipe for water irrigation system at Camden Bicentennial Equestrian Park when the pipe moved over the star picket and struck him on the head, causing fatal injuries.
After a SafeWork NSW investigation, the defendant, Camden Council, was charged with breaches of section 32/19(1) of the Work Health and Safety Act 2011.
On 21 December 2021, the defendant was convicted by the District Court and fined $750,000.
Lewin Roofing Pty Ltd

On 18 March 2019, a worker was carrying roof sheets when he fell approximately 6.5 metres through an unprotected skylight onto an internal stair landing, resulting in fractures to neck, skull and vertebrae, at Port Macquarie.
After a SafeWork NSW investigation, the defendant Lewin Roofing Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 21 December 2021, the defendant was convicted by the District Court and fined $135,000.
Fumigate All Hours (NSW) Pty Ltd

On 23 May 2018, SafeWork NSW was notified by South Eastern Sydney Local Health District of a worker suffering paralysis and loss of vision due to being exposed to Methyl Bromide whilst fumigating and degassing shipping containers at Cronulla.
After a SafeWork NSW investigation, the defendant, Fumigate All Hours (NSW) Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 17 December 2021, the defendant was convicted by the District Court and fined $75,000.

On 6 November 2018, a formworker was installing a 4.7 metre long steel reinforcement bar into a wall panel on the roof of a building under construction when it came into contact with overhead powerlines, resulting in serious injuries, at Guildford.
After a SafeWork NSW investigation, the defendant, Stephen Thompson, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.
On 17 December 2021, the defendant was convicted by the District Court and fined $30,000.
Hubtex Australia Pty Ltd

On 2 March 2017, a worker sustained serious burns to his face, arm and fingers while undertaking the task of dismantling a forklift gearbox housing with a flammable break cleaner and an electric rattle gun, at Riverstone.
After a SafeWork NSW investigation, the defendant, Hubtex Australia Pty Ltd, was charged with a breach of section 32/19(1) and section 38 of the Work Health and Safety Act 2011.
On 8 December 2021, the defendant was convicted by the District Court and fined $120,000 for the breach of section 32/19(1) of the Act and $4,000 for the breach of section 38 of the Act.
