On 13 April 2018, an 18 year old car wash attendant suffered 35% burns to his body when the wet and dry vacuum cleaner he was using to clean up a liquid spill from the staff room floor exploded, at Ashfield.
After a SafeWork NSW investigation, the corporate defendants, JPS Car Wash Management Pty Ltd and SRS Star Management Pty Ltd (SRS), were each charged with breach of section 32/19(1) of the Work Health and Safety Act 2011, and the director of SRS Sarjeet Sidhu, was charged with breach of section 32/27(1) of the Act.
On 30 April 2021, the defendants were convicted and fined by the District Court. The corporate defendants were fined $300,000 each, and the director was fined $30,000.
Ultra Refrigeration Pty Limited and Romolo Prestia

On 25 October 2017, two apprentices suffered injuries while replacing a three phase circuit breaker on an air conditioning system at a KFC store in Woodbine.
After a SafeWork NSW investigation, the corporate defendant, Ultra Refrigeration Pty Limited, and the director, Romolo Prestia, were respectively charged with a breach of section 32/19(1) and section 32/27(1) of the Work Health and Safety Act 2011.
On 30 April 2021, the defendants were convicted and fined by the District Court. The corporate defendant was fined $150,000, and the director was fined $15,000.
DIC Australia Pty Ltd

On 7 December 2017, a 42 year old contractor was fatally injured when he was trapped by an anchor blade inside an industrial ink holding tank while cleaning, at Auburn. Two other workers were also injured attempting to assist the trapped contractor.
After a SafeWork NSW investigation, the defendant, DIC Australia Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 30 April 2021, the defendant was convicted by the District Court and fined $450,000.
Candal Investments Pty Ltd

On 28 May 2018, a meatworker working at a Bomaderry factory suffered a deep laceration to her right forearm caused by the sudden explosion of a component of a gas system she was using to vacuum seal packaged meat.
After a SafeWork NSW investigation, the defendant, Candal Investments Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 29 April 2021, the defendant was convicted by the District Court and fined $60,000.
S & T Services Pty Ltd

On 6 August 2017, three workers were injured when grillage failed, causing the tower crane being installed on top of the grillage to collapse and land on an apartment block across the road at Wolli Creek.
After a SafeWork NSW investigation, the defendant, S & T Services Pty Ltd, was charged with a breach of section 32/22(2) of the Work Health and Safety Act 2011.
On 29 April 2021, the defendant was convicted by the District Court and fined $225,000.
McConnell Dowell Constructors (Aust) Pty Limited

On 1 March 2017, a 32 year old worker suffered fatal crush injuries when two 3 tonne steel structures known as ‘headstocks’ toppled over on a barge and fell on him, at Barangaroo.
After a SafeWork NSW investigation, the defendant, McConnell Dowel Constructors (Aust) Pty Limited, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
The defendant was convicted by the District Court and on 8 April 2021 was fined $500,000.
