On 25 May 2019, a major formwork collapse at Gosford exposed number of workers to risk of death or serious injury.

After a SafeWork NSW investigation, the defendant, Akkari Group Pty Ltd, was charged with a breach of sections 38, 32/19(1), section 32/26(2) of the Work Health and Safety Act 2011. 

On 18 July 2022, the defendant was convicted by the District Court and fined $37,500 for the section 38 offence and $281,250 each for the sections 32/26(2) and 32/19(1) offences.
