On 12 April 2019, a worker was operating a John Heine Power Press when his chair slipped and his foot connected with the unguarded lever, activating the Press, which crushed his right-hand fingers, at Mount Druitt.
After a SafeWork NSW investigation, the defendant, Ampelite Australia Pty Ltd, was charged with a breach of section 32/19(1) and 38(1) of the Work Health and Safety Act 2011.
On 22 February 2022, the defendant was convicted by the District Court and fined $135,000 for the breach of section 32/19(1) of the Act and $12,000 for the breach of section 38 of the Act.
Dynamic Formwork Group Pty Ltd

On 5 February 2019, a worker sustained serious injury when he fell approximately 2.8 meters from a formwork deck at a construction site in Guildford onto a concrete slab impaling his leg on a vertical steel reo bar.
After a SafeWork NSW investigation, the defendant, Dynamic Formwork Group Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 14 February 2022, the defendant was convicted by the District Court and fined $45,000.
All Seasons (Aust) Gourmet Produce Pty Ltd and Skevos Kakias

On 24 December 2018, a worker suffered serious injury when his right hand came in contact with the rotating blade of a spinach cutter while clearing the blockage on a conveyor belt, at Marrickville.
After a SafeWork NSW investigation, the corporate defendant, All Seasons (Aust) Gourmet Produce Pty Ltd, and the director, Skevos Kakias, were respectively charged with breaches of section 32/19(1) and 32/27(1) of the Work Health and Safety Act 2011.
On 11 February 2022, the defendants were convicted and fined by the District Court. The corporate defendant was fined $112,500, and the director was fined $37,500.
