On 15 May 2019 a worker sustained fatal injuries when a deck on the trailer descended and crushed him at Kirrawee.
After a SafeWork NSW investigation the defendant, Autocare Services Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 16 December 2022, the defendant was convicted by the District Court and fined $450,000.
BSA Advanced Property Solutions (Fire) Pty Ltd

On 29 November 2019, a 49 year old fire system installer sustained fatal injuries when he uncapped a pressurised water pipe resulting in the test cap on the end of the pipe ejecting with considerable force striking him in the head, at Sydney.
After a SafeWork NSW investigation the defendant, BSA Advanced Property Solutions (Fire) Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 16 December 2022, the defendant was convicted by the District Court and fined $450,000.

On 20 November 2019, a worker was seriously injured whilst unpacking pallets of copper cable inside an elevated shipping container when the top pallet weighing approximately 500kg fell on him and he fell 1.5 metres to the ground, at Newington.
After a SafeWork NSW investigation, the defendant, Cincram Group Pty Ltd was charged with a breach of section 32/19(1) of the Work Health Safety Act 2011.
On 9 December 2022, the defendant was convicted by the District Court and fined $375,000

On 19 August 2019 a 28 year old worker sustained serious injuries with severe burns to his face, neck and ears from a fire and/or explosion occuring out of a road crack sealing machine at Jambaroo.
After a SafeWork NSW investigation the defendant, RoadWorx Surfacing Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 9 December 2022, the defendant was convicted by the District Court and fined $340,000.
Tyne ACFS Pty Ltd

On 25 September 2019, a 67 year old male sustained fatal injuries when he was caught between two shipping containers, at Eastwood.
After a SafeWork NSW investigation, the defendant, Tyne ACFS Pty Ltd was charged with a breach of section 32/19(1) of the Work Health Safety Act 2011.
On 7 December 2022, the defendant was convicted by the District Court and fined $300,000.
