On 19 August 2020, a section of a scaffold surrounding the site collapsed due to wind onto a pedestrian footpath striking a member of the public, damaging vehicles and a light pole.
After a SafeWork NSW investigation the defendant, Civil 1 Pty Limited, was charged with a breach of section 197 of the Work Health and Safety Act 2011.
On 27 June 2025, the defendant was convicted by the District Court and fined $90,000.
Mazin Abdel Samad

In August 2023, Mr Samad is alleged to have not complied with a s155 notice issued upon him for information in relation to a SafeWork investigation.
After a SafeWork NSW investigation the defendant, Mazin Abdel Samad, was charged with a breach of section 155/(5) of the Work Health and Safety Act 2011.
On 2 June 2025, the defendant was convicted by the Downing Centre Local Court and fined $1,100.
