On 27 January 2021 a worker sustained fatal injuries when the cables of a crane used to lift a submerged yacht broke and fell onto him.
After a SafeWork NSW investigation the defendant, AWB Contractors Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 23 July 2025 the defendant was convicted by the District Court and fined $765,000.
Bellabrae Homes Pty Ltd

On 9 June 2022, a 19 year old plumbing apprentice, was clearing out a trench with a shovel when a 2m high unsupported excavation collapsed on him.
After a SafeWork NSW investigation the defendant, Bellabrae Homes Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011 (WHS Act).
On 11 July 2025, the defendant was convicted by the District Court and fined $225,000 and imposed a Publicity order under section 236 of the WHS Act.
Duralina Pty Ltd

On 3 September 2021, a 38 year old worker sustained serious crush injury when his hand was caught in fine rollers whilst attempting to restart a mill.
After a SafeWork NSW investigation the defendant, Duralina Pty Ltd, was charged with a breach of section 32/21(2) of the Work Health and Safety Act 2011.
On 11 July 2025, the defendant was convicted by the District Court and fined $56,250.
Harbour City Roofing Pty Ltd

On 9 February 2021, a 25 year old worker was seriously injured when he fell 5.5m through a roof structure to the ground below.
After a SafeWork NSW investigation the defendant, Harbour City Roofing Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 10 July 2025, the defendant was convicted by the District Court and fined $600,000.
SG Interior Linings Pty Ltd

On 6 May 2022, a 38 year old worker was working in a window portal on the first floor of a refurbished library when he fell 5m resulting in head and spinal injuries.
After a SafeWork NSW investigation the defendant, SG Interior Linings Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 10 July 2025, the defendant was convicted by the District Court and fined $90,000.
465 Leichhardt Pty Ltd

On 2 September 2022 a 33 year old woman and 3 small children sustained serious injuries when a gate came out of the fence frame and fell onto them.
After a SafeWork NSW investigation the defendant, 465 Leichhardt Pty Ltd, was charged with a breach of section 32/20(2) of the Work Health and Safety Act 2011.
On 2 July 2025, the defendant was convicted by the Industrial Court of NSW and fined $180,000.
