On 25 October 2018, a prohibition notice was issued to prevent workers using incomplete scaffold. The notice was contravened when workers continued to work on the scaffold prior to the notice being complied with, at Marsden Park.
After a SafeWork NSW investigation, the defendant, AAA Rapid Homes Pty Ltd, was charged with a breach of clause 79(2) of the Work Health and Safety Regulation 2017 (the Regulation) and section 197 of the Work Health and Safety Act 2011 (the Act).
On 27 September 2019, the defendant was convicted by the Local Court and fined $30,000 for section 197 of the Act and $10,000 for clause 79(2) of the Regulation.
MGK Constructions Pty Ltd as trustee for The Keramea Family Trust

On 19 May 2017, a 32 year old first year apprentice carpenter suffered serious injury when he fell through an unprotected stair void at a construction site in St Ives.
After a SafeWork NSW investigation, the defendant, MGK Constructions Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 23 September 2019, the defendant was convicted by the District Court and fined $75,000.
Adnan Yassine

On 01 March 2016, excavation work at a construction site compromised the structural integrity of the surrounding buildings, and one building was partly demolished as a result of the excavation, at Enfield.
After a SafeWork NSW investigation, the defendant, Adnan Yassine, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.
On 06 September 2019, the defendant was convicted by the District Court and fined $30,000.

On 23 January 2016, a worker was seriously injured while undertaking scheduled maintenance work on a gearbox, that drives a mill, when a 700kg steel housing fell on his lower body, at Maldon.
After a SafeWork NSW investigation, the defendant, K & R Fabrication (W’Gong) Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.
On 03 September 2019, the defendant was convicted by the District Court and fined $75,000.
