Yan Huai (aka Anne) Wu

On 4 February 2016, a 51 year old machine operator suffered amputation of four fingers when she was using an unguarded industrial pillow filling machine.

After a SafeWork NSW investigation, the defendant, Yan Huai Wu, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 30 July 2018, the defendant was convicted by the District Court, fined $7,500, and ordered to complete 24 hours of training under a s.241 Order.

Zenger (Aust) Pty Ltd

On 4 February 2016, a 51 year old machine operator suffered amputation of four fingers when she was using an unguarded industrial pillow filling machine.

After a SafeWork NSW investigation, the defendant, Zenger (Aust) Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 30 July 2018, the defendant was convicted by the District Court and fined $60,000.

Turfco Australia Pty Ltd

16 July 2018

On 1 December 2014, a 19 year old worker suffered fatal crush injuries when he fell from the side platform of a harvester which was travelling in reverse.

After a SafeWork NSW investigation, the defendant, Turfco Australia Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 16 July 2018, the defendant was convicted by the District Court and fined $375,000.

Ru Dong Li

13 July 2018

On 24 June 2016, a 58 year old worker suffered fatal injuries when he fell approximately 2.4 metres from a ladder while installing a security camera.

After a SafeWork NSW investigation the defendant, Ru Dong Li was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 13 July 2018, the defendant was convicted by the District Court and fined $60,000.

Shane Devine

11 July 2018

On 5 July 2016, an asbestos roof was water blasted as part of a roof painting contract at a residential property by the defendant and his brother. The water blasting contaminated the property and neighbouring properties with friable asbestos.

After a SafeWork NSW investigation the defendant, Shane Devine, was charged with a breach of section 33/19(2) of the Work Health and Safety Act 2011.

On 6 July 2018, the defendant was convicted by the District Court and fined $200,000.

Cosentino Australia Pty Ltd

6 July 2018

On 11 November 2014, a worker was seriously injured when he was trapped by granite slabs weighing approximately 1.6 tonnes as they were being removed from an "A" frame transport stand.

After a SafeWork NSW investigation the defendant, Cosentino Australia Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 6 July 2018, the defendant was convicted by the District Court and fined $200,000.
