On 19 July 2016, a 32 year old worker suffered injuries to her toe when a forklift ran over her left foot.

After a SafeWork NSW investigation, the defendant, Williams Pressing & Packaging Services Pty Ltd was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 20 December 2018, the defendant was convicted by the District Court and fined $60,000.

Cilin Cai

On 10 September 2016, a 55 year old worker at Strathfield fell approximately three metres, head first from an unprotected edge at a construction site.

After a SafeWork NSW investigation, the defendant, Cilin Cai, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 14 December 2018, the defendant was convicted by the District Court and fined $24,000.

Grandcity Constructions Pty Ltd

On 10 September 2016, a 55 year old worker at Strathfield fell approximately three metres head first from an unprotected edge at a construction site.

After a SafeWork NSW investigation, the defendant, Grandcity Constructions Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 14 December 2018, the defendant was convicted by the District Court and fined $120,000.

Shaun David Scott Mehan

On 24 April 2015, a 25 year old apprentice at Miranda suffered serious injuries when he was moving a 530kg air conditioning unit down the stairs with other workers when they lost control and the unit fell on him.

After a SafeWork NSW investigation, the defendant, Shaun Mehan, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 13 December 2018, the defendant was convicted by the District Court and fined $7,500.

Cleo Antoniou

On 3 August 2012 (notified 22 July 2015), a 22 year old worker suffered serious injuries to her hand when it became trapped in the manual cup folding machine she was operating at Marrickville.

After a SafeWork NSW investigation, the defendant, Cleo Antoniou, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 13 December 2018, the defendant was convicted by the District Court and fined $25,500.

Confeta Pty Ltd

On 3 August 2012 (notified 22 July 2015), a 22 year old worker suffered serious injuries to her hand when it became trapped in the manual cup folding machine she was operating at Marrickville.

After a SafeWork NSW investigation, the defendant, Confeta Pty Ltd, was charged with a breach of section 32/19(1), section 38(1) and section 39(1) of the Work Health and Safety Act 2011.

On 13 December 2018, the defendant was convicted by the District Court and fined $150,000 under section 32/19(1) of the Act; $3,750 under section 38(1) of the Act and $3,750 under section 39(1) of the Act.

International Oils Machinery (Aust) Pty Ltd T/A Coffs Harbour Paper and Oil

On 6 November 2015, a 50 year old worker at Toormina was injured when his arm was drawn into a machine at a recycling plant and was trapped for several hours.

After a SafeWork NSW investigation, the defendant, Internationals Oil Machinery (Aust) Pty Ltd T/A Coffs Harbour Paper and Oil, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 10 December 2018, the defendant was convicted by the District Court and fined $15,000.

Hiltour Pty Ltd

On 19 January 2016, a 29 year old compounder suffered serious chemical burns to his left leg while decanting a hazardous chemical, known as Compset DER 731 Epoxy Diluent (DER 731), at West Gosford.

After a SafeWork NSW investigation, the defendant, Hiltour Pty Ltd, was charged with a breach of section 32/19(1) and section 38(1) of the Work Health and Safety Act 2011.

On 3 December 2018, the defendant was convicted by the District Court and fined $45,000 under section 32/19(1) of the Act and $3,750 under section 38(1) of the Act.

MPMG FM Pty Ltd

On 28 April 2016, two painters at Heathcote suffered serious injuries when the mobile scaffolding they were working from collapsed.

After a SafeWork NSW investigation, the defendant, MPMG FM Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 3 December 2018, the defendant was convicted by the District Court and fined $90,000.

Chiew Meng Ng

Between February 2016 and 7 June 2017 SafeWork identified that a development undergoing construction at Chester Hill had failed to address work health and safety issues arising from incomplete scaffolding, poor housekeeping and inadequate site security.

After a SafeWork NSW investigation, the defendant, Chiew Meng Ng, was charged with three breaches of section 33/27(1) of the Work Health and Safety Act 2011.

On 28 November 2018, the defendant was convicted by the Local Court and fined a total of $42,500.

PIA Constructions and Engineering Pty Ltd

Between February 2016 and 7 June 2017 SafeWork identified that a development undergoing construction at Chester Hill had failed to address work health and safety issues arising from incomplete scaffolding, poor housekeeping and inadequate site security.

After a SafeWork NSW investigation, the defendant, PIA Constructions and Engineering Pty Ltd, was charged with a total of nine offences under section 33/19(1), section 22/19(2) and section 33/20(2) of the Work Health and Safety Act 2011.

On 28 November 2018, the defendant was convicted by the Local Court and fined a total of $160,000 under section 19(1) of the Act, a total of $45,000 under section 19(2) and $5000 under section 20(2) of the Act.

Houssam Annous

On 31 January 2016, a worker at Greenacre was fatally injured when the asbestos roof sheeting of the warehouse he was working on broke, causing him to fall approximately 8.7 metres to the ground.

After a SafeWork NSW investigation, the defendant, Houssam Annous, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 23 November 2018, the defendant was convicted by the District Court and fined $7,500.

Opcon Plumbing Pty Ltd

On 31 January 2016, a worker at Greenacre was fatally injured when the asbestos roof sheeting of the warehouse he was working on broke, causing him to fall approximately 8.7 metres to the ground.

After a SafeWork NSW investigation, the defendant, Opcon Plumbing Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 23 November 2018, the defendant was convicted by the District Court and fined $75,000.

On 18 May 2016, two workers suffered serious burns during a fire at a waste storage facility. The fire started when a crushing unit caused a pallet of aerosol cans to explode at Auburn.

After a SafeWork NSW investigation, the defendant, Enviro Services Group Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 18 October 2018, the defendant was convicted by the Local Court and fined $50,000.

On 12 January 2016, a 48 year old Labourer was injured when he fell through a void on the third level of a building under construction which was not adequately covered.

After a SafeWork NSW investigation, the defendant, Rock Form Group Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 14 September 2018, the defendant was convicted by the District Court and fined $127,500.

On 12 January 2016, a 48 year old Labourer was injured when he fell through a void on the third level of a building under construction which was not adequately covered.

After a SafeWork NSW investigation, the defendant, Deicorp Pty Ltd, was charged with a breach of of section 32/19(1) of the Work Health and Safety Act 2011.

On 14 September 2018, the defendant was convicted by the District Court and fined $75,000.

On 5 July 2016, an asbestos roof was water blasted as part of a painting contract at a residential property by the defendant and his brother. The water blasting contaminated the property and neighbouring properties with friable asbestos.

After a SafeWork NSW investigation, the defendant, Matthew Devine, was charged with a breach of section 33/19(2) of the Work Health and Safety Act 2011.

On 12 September 2018, the defendant was convicted by the Local Court and fined $18,000.

On 25 November 2015, a tree being felled at a private residence fell in an uncontrolled manner, causing significant damage to the roof and front fa├žade of a neighbouring house.

After a SafeWork NSW investigation, the defendant, Daniel Rose, was charged with a breach of section 32/19 (1), section 32/19(2) and section 38 of the Work Health and Safety Act 2011.

On 31 August 2018, the defendant was convicted by the District Court and fined $7,500 under section 32/19(1), $7,500 under section 32/19(2), and $1,500 under section 38 of the Work Health and Safety Act 2011.

On 4 October 2014, a general factory hand suffered crush injuries to his left hand and partial amputation of four finders while operating a hydraulic press brake machine.

After a SafeWork NSW investigation, the defendant, Duct Australia Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 27 August 2018, the defendant was convicted by the District Court and fined $112,500.

On 8 October 2014, a 49 year old fumigator was fatally injured while assisting to remove a tyre on a container loader. As the tyre ruptured, the tyre and outer rim struck the fumigator, causing fatal injuries.

After a SafeWork NSW investigation, the defendant, Haconby Pty Ltd, was charged with a breach of section 32/19(1) and section 32/19(2) of the Work Health and Safety Act 2011.

On 27 August 2018, the defendant was convicted by the District Court and fined $75,000 under section 32/19(1) and $225,000 under section 32/19(2) of the Work Health and Safety Act 2011.

On 4 February 2016, a 51 year old machine operator suffered amputation of four fingers when she was using an unguarded industrial pillow filling machine.

After a SafeWork NSW investigation, the defendant, Yan Huai Wu, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 30 July 2018, the defendant was convicted by the District Court, fined $7,500, and ordered to complete 24 hours of training under a s.241 Order.

On 4 February 2016, a 51 year old machine operator suffered amputation of four fingers when she was using an unguarded industrial pillow filling machine.

On 16 July 2018, the defendant was convicted by the District Court and fined $375,000.

On 24 June 2016, a 58 year old worker suffered fatal injuries when he fell approximately 2.4 metres from a ladder while installing a security camera.

After a SafeWork NSW investigation the defendant, Ru Dong Li was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 13 July 2018, the defendant was convicted by the District Court and fined $60,000.

On 6 July 2018, the defendant was convicted by the District Court and fined $200,000.

On 24 September 2015, a 20 year old labourer and a 19 year old labourer, suffered disorientation and vomiting after cleaning inside a metal pontoon, a confined space with no ventilation while using paint thinners.

After a SafeWork NSW investigation the defendant, Central Coast Metal Protectives Pty Ltd, was charged with a breach of section 32 / 19(1) of the Work Health and Safety Act 2011.

On 22 June 2018, the defendant was convicted by the District Court and fined $45,000.

On 28 May 2018, the defendant was convicted by the Local Court of 1 count pursuant to s43(1) of the Work Health and Safety Act and fined $6000 with 4 matters taken on a Form 1, and 1 count pursuant to s268(2) of the Work Health and Safety Act with 1 matter taken on a Form 1 and fined $3000.

After a SafeWork NSW investigation the defendant Mark Marando was charged with a breach of section 193 of the Work Health and Safety Act 2011.

On 18 May 2018, the defendant was convicted by the Local Court and fined $2,000.

On 20 October 2015 a 27 year old excavator operator, and David Giles Armstrong, a 54 year old plumber suffered serious injuries while attempting to repair a ruptured natural gas line. Mr Armstrong operated an electric jack hammer near the rupture which ignited the natural gas.

After a SafeWork NSW investigation the defendant David Giles Armstrong was charged with a breach of section 32/28 of the Work Health and Safety Act 2011.

On 30 April 2018, the defendant was convicted by the Local Court and fined $2,200.

On 29 March 2018, the defendant was convicted by the District Court and fined $80,000.

On 30 June 2015, a worker was fatally injured when he fell 2.9 metres onto a concrete slab while installing a walking platform between roof trusses.

After a SafeWork NSW investigation the defendant, Co-Wyn Building Contractors Pty Ltd, was charged with a breach of section 32 / 19(1) of the Work Health and Safety Act 2011.

On 26 March 2018, the defendant was convicted by the District Court and fined $405,000.

On 8 January 2015, a worker was seriously injured when he fell approximately 5 meters from a roof when the temporary edge protection he leant on gave way.

After a SafeWork NSW investigation the defendants, Edgesafe Pty Ltd and Christopher Butler were respectively charged with a breach of section 32/26(2) and 32/27(1) of the Work Health and Safety Act 2011.

On 26 March 2018, the defendants were convicted by the District Court. The corporate defendant was fined $75,000 and the director defendant was fined $5,500.

On 8 April 2015, a 47 year old female customer died during a drift snorkeling tour after getting into difficulties in a strong current.

After a SafeWork NSW investigation, the defendants, Millart Enterprises Pty Ltd and Notlad Enterprises Pty Ltd, were each charged with a breach of section 32/19(2) of the Work Health and Safety Act 2011.

On 19 March 2018, both defendants were convicted by the District Court and fined $60,000 each.

On 2 October 2015 a worker suffered crush injuries when the contents of a shipping container fell on him while he was unloading the container.

After a SafeWork NSW investigation the defendants, Modern Touch Marble and Granite Pty Ltd and its Director Johnne Khouri, were respectively charged with a breach of section 32/19(1) and 32/27(1) of the Work Health and Safety Act 2011.

On 20 February 2018 the defendants were convicted by the District Court and the corporate defendant fined $75,000 and the director defendant fined $12,500.

Universal Property Group Pty Ltd

On 16 February 2015 a 21 year old Surveyor's Assistant was injured when he fell four meters through a penetration which was not adequately covered, striking some of the steel reinforcing bars. The penetration was covered with a piece of plywood that did not identify a penetration was below.

After a SafeWork NSW investigation the defendant, Universal Property Group Pty Ltd, was charged with a breach of Section 32/19(1) of the Work Health and Safety Act 2011.

On 20 February 2018 the defendant was convicted by the District Court and fined $135,000.

Ashley Vernon Errol Powell

On 1 June 2015 a worker was injured when he was hit by a swinging branch cut from a tree causing him to fall approximately 11 metres from the roof of a house.

After a SafeWork NSW investigation the defendant, Ashley Vernon Errol Powell, was charged with a breach of section 32/27(1) of the Work Health and Safety Act 2011.

On 12 February 2018 the defendant was convicted by the District Court and fined $20,000.

H&F Mechanical Pty Ltd, Max Alfred Hoffman and Laurence James Hoffman

Between 1 February 2014 to 16 May 2014 workers were exposed to a risk of serious injury in the operation of a self-contained wood-processing unit.  The manufacturer’s built-in safety device had been removed, being the double handed system used to operate the machine, and modified to permit an operator to use the handles with one hand instead of two. Those modifications were known to each of the three defendants.

After a SafeWork NSW investigation the defendants H & F Mechanical Pty Ltd, Max Alfred Hoffman and Laurence James Hoffman, were each charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 2 February 2018 the defendants were convicted by the District Court and the corporate defendant was fined $160,000 and the individual defendants were each fined $32,000.

Broadspectrum (Australia) Pty Ltd

On 29 January 2015 a 66 year old boilermaker suffered fatal injuries when a corroded storage rack collapsed towards him while he was attempting to access or retrieve materials from the storage rack.

After a SafeWork NSW investigation the defendant, Broadspectrum (Australia) Pty Ltd, was charged with a breach of section 32/19(1) of the Work Health and Safety Act 2011.

On 2 February 2018 the defendant was convicted by the District Court and fined $240,000.
