Worker fatally injured while working near mobile plant (6 November 2025)
A truck driver was performing cleaning activities at the rear of his vehicle when a shuttle buggy, operated by another worker, struck his truck. This pinned the truck driver between the two vehicles, causing fatal injuries.

Consider ‘reasonably practicable’ control measures to manage the risks associated with working around moving plant.
Ensure:
- you eliminate traffic risks by implementing exclusion zones
- dedicated safe wash-down areas are available
- you use directional signage such as ‘workers ahead’ and speed reduction signs
- you install delineation devices such as traffic cones, temporary bollards and barrier boards to define the traffic path through, past or around work areas
- before operating plant, you look in all directions for pedestrians, are aware of any exclusion zones and/or areas where pedestrians and other work tasks are being performed on site
- there is adequate communication between plant operators and workers on foot
- a warning device must be used when there is a possibility of plant coming into contact with pedestrians or other mobile plant
- plant you are operating is fitted with flashing lights, sensors and reversing alarms
- vehicles and mobile plant are inspected and maintained in accordance with the manufacturer’s recommendations
- a site-specific Safe Work Method Statement is in place, and all workers have a site-specific induction prior to commencing work
- workers have the necessary training, experience and supervision to identify hazards and control the risks associated with working around and operating moving plant
- appropriate personal protective equipment is available and always used
- emergency and first aid plans are in place and staff are trained.
