Young worker fatally injured after impalement on a starter bar (16 October 2025)
A young worker was performing duties at a residential construction site and subsequently was impaled on an uncapped vertical starter bar.

Consider ‘reasonably practicable’ control measures to manage the risks associated with uncapped starter bars and star pickets'
Ensure:
- all exposed starter bars and star pickets are protected with adequate controls (such as bar and picket caps) at the time of installation and remain in place whilst work is occurring.
- guardrails, screens or other means are used to separate workers from exposed bars
- designated walkways are in place that are free from slip, trip and fall hazards
- adequate lighting and signage are implemented on worksite
- the site is kept clean and free from unwanted and unnecessary materials
- a designated area is maintained for storing construction materials
- consultation is done with workers each time the site conditions change. This can be done via regular pre-starts or daily toolbox talks
- control measures are in place to eliminate or minimise the risk of being impaled
- young and inexperienced workers receive adequate information, training and supervision, with consideration given to their age, experience and competence levels
- a safe system of work is in place with an exclusion zone to prevent persons from being physically able to come in contact with starter bars in the event they slip, trip or fall
- comprehensive emergency plans are prepared, maintained and implemented to provide an effective response in the event of an incident.
