Electrician Received Fatal Electric Shock During UPS Commissioning (24 November 2025)
During UPS commissioning work, an electrician was installing meter probes on uninsulated copper bars inside a switchboard in preparation for testing supply with a power quality meter. While installing meter probes, the electrician came into contact with energised components and sustained a fatal electric shock.
Safety information
In NSW, it is prohibited to work on live electrical equipment unless one or more exceptions apply. When work is carried out or near electrical equipment, duty holders must implement control measures to eliminate risks to health and safety so far as is reasonably practicable. If it is not reasonably practicable to eliminate risks to health and safety, minimise the risks so far as is reasonably practicable.
Ensure:
- all energy sources are identified, isolated, and electrical equipment is tested by a licenced electrician to ensure it is de-energised prior to commencing work
- the isolation switch of all potential sources is locked out / tagged out (LOTO) to prevent any unexpected startup or inadvertent contact with live parts
- electrical work is carried out from a position that minimises the risk of inadvertent contact with exposed energised parts
- work in tight or awkward spaces is avoided where possible, reduce risk through adequate separation, and apply appropriate insulation
- equipment is checked for damaged covers, cords, exposed wires, faulty power switches and circuit breakers, scorch marks and burning smells
- the person who manages or controls the workplace is consulted with about timing of the work and all parties who may be affected are notified
- high-voltage exposed parts are earthed after proven to be de-energised
- a site-specific safe work method statement (SWMS) is in place that outlines the hazards, risks and control measures of work being performed
- all workers are provided and use appropriate tools, rescue equipment, and personal protective equipment
- all test equipment is periodically calibrated in accordance with manufacturer’s recommendation
- comprehensive emergency plans are in place and workers are trained in their implementation.
