Fatal farm incident involving a side-by-side vehicle (SSV) (21 January 2026)
A worker was planning to conduct weed spraying on a rural property using a side-by-side all-terrain vehicle (ATV). The worker was later found trapped underneath the vehicle which had rolled down a two-metre-high creek bank and onto its side. At the time of the incident the worker was not wearing the seatbelt fitted to the ATV. The worker sustained fatal injuries.

Consider ‘reasonably practicable’ control measures to manage the risks associated with side-by-side vehicles and all-terrain vehicles.
Ensure:
- vehicles selected for work tasks are fit for purpose and provide the highest level of protection for the operator and any passengers
- built-in safety features, including rollover protection structures (ROPS), seat belts, and cab nets or doors, are maintained in working order and used at all times
- all occupants wear seat belts when operating side-by-side vehicles
- operators and passengers wear helmets where reasonably practicable or recommended by the manufacturer or risk assessment
- cab nets or doors are closed and secured during operation to prevent occupants’ limbs from protruding outside the vehicle in the event of a rollover
- passengers are only carried in designated seating positions and never in rear cargo trays
- operators are trained and competent in the safe operation of side-by-side vehicles, including understanding vehicle limitations and rollover risks
- the safest route is selected before operating the vehicle, taking into account terrain, slopes, surface conditions, and potential obstacles
- systems of work are in place to prevent vehicle rollover and crush injuries, including supervision and task planning
- operators never exceed recommended load limits
- vehicles are in park mode with hand brake activated when not in use
- all side-by-side and All-Terrain vehicles are maintained in accordance with manufacturers’ instructions.
